삼차신경통으로 진단된 하치조신경의 절제와 조직변성

Abstract
결론
약물 및 보존적 치료에 효험이 없어 난치성 삼차신경통으로 진단된 악안면부 동통 환자에
서 진단적 전달마취를 시행하여 신경변성을 예측하고, 하치조신경을 부분절제하여 특수염색
검경 및 전자현미경으로 조직병리학적 검사를 하였으며, 술후의 경과를 4년이상 관찰하여
다음과 같은 결과를 얻었다.
1. 약물요법에 효과가 없는 삼차신경통은 신경내변성(endoneural degeneration)을 보였고,
인지하지 못한 삼차신경 손상의 퇴행성 변화의 결과로 사료된다.
2. 하악관내 하치조신경의 절제는 하악지의 시상분열골절단술(SSRO)로 접근하고 3cm 이
상의 확실한 절제가 요한다.
3. 신경절제후 동통은 소실되었으나, 50%에서 술후 2∼3년에 동통의 재발이 있었고, 이는
절단 상부 및 장협신경에서 새롭게 분지한 collateral neuropathy로 검경되었다.
4. 술후 재발된 동통은 익돌하악-하악후구치부 신경종성 반흔조직의 부가적 제거후에 소
실되었고 다시 재발하지 않았다.
5. 절제된 신경은 특수염색의 광학현미경 관찰에서 myelin의 감소와 신경속내 섬유화증식
이 현저하였다.
6. 전자현미경 검경에서는 신경축색의 파괴, myelin의 연속성 상실과 변성, Schwann cell
의 변성이 관찰되었다.
#초록#
This is to report the histo-pathologic findings of the degenerated inferior alveolar
nerves(IAN) causing uncontrollable neuralgia persisted for over 3 years.
The patients had sufferred from frequent attack of unbearable pain along the
mandibular branches of the Vth nerve. The pain arised spontaneously 5-10 times a day
and lasted for 2-5 minutes. The medications including Tegretol were
neither so effective to alleviate the pain level, nor enough to keep free from pain all
days.
The peripheral neurectomies were preceded by diagnostic nerve block, and partial
resection of the IAN in the mandibular canals were approached by sagittal split osteomy
of the ramus.
The histopathologic findings of the resected IAN were as follows ;
1. Special staining with Luxol Fast Blue (LFB) and Masson Trichrome (MT) disclosed
marked degeneration of the axons, decreased in number and fibrosis between the nerve
fibers.
2. EM study revealed destruction of myelin continuity surrounding axons, degeneration
of Schwann cell and endoneureal sheath, and coarse collagen between the destructed
axons.
3. The 3 of 6 patients presented again with recurred pain in 2-3 years after the
peripheral neurectomies. Excisional biopsy of the retromolarpterygomandibular softtissues
disclosed the collateralization neuropathy beside the long-buccal nerves and their
neuromatous findings.
